    It seems obvious that #prevention and #communitycare will improve our health and reduce the demand for hospital and GP services. But making the 'business case' requires high quality economic analysis. Really interesting work from the NHS Confederation today exploring these links. However, more data and analysis is required to unpick causality and help local #ICS #NHS leaders to make informed decisions about their spending priorities. https://lnkd.in/eHiq_EtU

    CHIA has published its latest report on health care equity in Massachusetts, 𝘏𝘰𝘴𝘱𝘪𝘵𝘢𝘭 𝘜𝘵𝘪𝘭𝘪𝘻𝘢𝘵𝘪𝘰𝘯 𝘪𝘯 𝘔𝘢𝘴𝘴𝘢𝘤𝘩𝘶𝘴𝘦𝘵𝘵𝘴: 𝘈𝘯 𝘈𝘴𝘴𝘦𝘴𝘴𝘮𝘦𝘯𝘵 𝘣𝘺 𝘙𝘢𝘤𝘦 & 𝘌𝘵𝘩𝘯𝘪𝘤𝘪𝘵𝘺. This report assesses differences in acute care utilization by race and ethnicity for Massachusetts residents, including inpatient discharges and emergency department visits from acute care hospitals, for state fiscal year (SFY) 2021. Using data from CHIA’s Acute Hospital Case Mix Database (Case Mix), this report is part of a series exploring health care equity in the Commonwealth. While Massachusetts residents continue to face challenges accessing high quality, affordable, and timely health care, the systemic inequities and institutional racism faced by historically marginalized groups further exacerbate these issues. To advance the goal of health care equity in the Commonwealth, it is imperative to prioritize the collection and analysis of reliable and meaningful health care information on subpopulations to inform policymakers, health care providers, payers, and other industry partners.     🏥 Rural healthcare’s outlook just worsened, according to a new analysis from Chartis, a healthcare advisory firm.The updated analysis of key indicators such as rural hospital operating margin, facility closures, and loss of access to care and services paints a grim picture for rural hospitals in the US, particularly independent providers.Half of rural hospitals are operating in the red, the analysis found, and that percentage increased from 43 percent a year ago. More independent rural hospitals are operating at a loss at 55 percent, while 42 percent of health system-affiliated rural hospitals have a deficit.     Collier County is designated as a Medically Underserved Area (MUA) as recognized by the US Health Resources and Services Administration (HRSA). This designation stems from a shortage of primary healthcare services available to its residents. The ramifications of this classification ripple throughout the community, particularly impacting access to healthcare for children and families. This translates to prolonged wait times for appointments, limited availability of preventative care services and heightened reliance on emergency room visits for basic medical needs.Consequently, children and families face significant barriers in obtaining timely and comprehensive healthcare, exacerbating existing health disparities and compromising overall well-being. The uninsured or underinsured population encounter substantial obstacles in affording necessary medical treatments. Addressing these challenges demands concerted efforts to expand access and enhance outreach initiatives aimed at underserved populations. Only through collaborative endeavors can Collier County begin to bridge the healthcare gap and ensure equitable access to essential services for all its residents.

    Primary health careThe definition of primary healthcare has evolved severally since its conception in 1978. The development of modern technology and growing economies led to an imbalance in the quality of healthcare provided in the regions of countries. This inequality drove the revaluation of primary healthcare to respond to the healthcare needs of communities. WHO and UNICEF also came up with a straightforward definition that guides the implementation of primary healthcare at the global, national, and regional levels. Primary healthcare can be defined as a community approach to health that guarantees the highest possible level of health and wellbeing and equal distribution of services by addressing the community’s needs and providing care early along the continuum of health promotion and disease prevention, including treatment, rehabilitation, and palliative care, at centres located as close as possible to people’s everyday environments. The structure of primary health care comprises three related components. These include comprehensive health services focused on primary care and the provision of public health goods and functions, multi-sectoral directives and policies that tackle the indirect determinants of health, and programmes that engage and empower community members to enhance social participation, self-care, and self-reliance.Furthermore, primary healthcare is grounded on equity, justice, unity, and participation. It recognises that the highest possible standard of health should be considered a basic human right. Primary healthcare forms a unique aspect of the public health sector as it seeks to resolve and maintain physical, mental, and social wellbeing. It also emphasises the vast determinants of health outside the physical realm. Perhaps none are more challenging than the conversation between #providers and their #patients regarding end-of-life scenarios. Koda Health is changing that.“Our mission is that advance care planning should be available for everyone, regardless of age, socioeconomic status, or the severity or progression of their disease state,” said Tatiana Fofanova, Ph.D., CEO, Koda Health which was recently named a finalist in the @Digital Health Hub Foundation and Digital Health Awards #Longevity category.Koda has built a shared decision making and education platform targeting health systems and payors, enabling their patients to complete the process of proactive healthcare planning, auto-generateing legal documents to protect their care, and virtually notarizing these documents at no cost to these patients.“We are able to help patients plan for their future health care. That impacts their treatments, and their quality of life now, to and through serious illness, and all the way up to the end of life,” Fofanova said.Advance care planning can be extremely painful and challenging for patients and their families. It can also impact providers who, based on our current standards for care, know they are delivering care that may be unnecessary and that patients might not want. It also can have a direct financial impact on health plans, value-based providers and others.“At the end of the day, it affects risk-bearing entities and payors that are paying for that delivery of care that patients might not want,” Fofanova said.While there is an awareness of the need for impactful #ACP, the industry is bogged down with antiquated and ineffective approaches - with many still relying on paper forms. This leads to relatively low engagement, with a frustrating and forgettable few minutes of checking boxes for participating patients. Such approaches yield more problems than benefits for the healthcare organizations. Because its solution solves these inefficencies, Koda is seeing tremendous market traction.
